Free Million Hearts® Health Screening Events Set for Buffalo Churches, Community Centers

Millennium Collaborative Care, PPS, in partnership with Greater Buffalo United Ministries, the University at Buffalo School of Nursing, will be hosting five Million Hearts® health screening events throughout the month of April at Buffalo-area churches and community centers. Adults 18-years of age and older can visit the following for free health screenings:

In addition to blood pressure screening by UB Nursing students, attendees will visit five additional stations including, Personal Health Survey; Height and Weight, along with education and resources for nutrition and exercise. “Cardiovascular disease which includes heart attacks and strokes is the leading reason for death across our eight Western New York counties, ” said Kenneth Lee Gayles. M.D., Cardiologist, Gayles Medical; and Project Champion, Cardiovascular Disease/Million Hearts®. “The goal really is to control blood pressure, because blood pressure If not treated, can result in people having heart attacks and strokes,” added Priti Bangia, Population Health Manager, Millennium Collaborative Care.


After completing their testing and screenings, a community navigator will also assist attendees with checking their eligibility for Medicaid, as well as help them find a primary care provider if they currently do not have one.


Heart disease and stroke are some of the top leading causes of death in the U.S. Statistics show that every 43 seconds someone has a heart attack in the U.S., as well as 1 American dies every 4 minutes from a stroke, according to Million Hearts® is a national initiative working to prevent cardiovascular disease among Americans and improve access to proper care. They work to enhance the quality of life by using the ABCS heart health guidelines. These guidelines include, Aspirin when appropriate; Blood pressure control; Cholesterol management; and Smoking cessation.


Igniting Healthcare Change in Western New York Millennium Collaborative Care (MCC), is a Performing Provider System (PPS) under New York State’s Delivery System Reform Incentive Payment (DSRIP) program, a statewide initiative championed by Governor Andrew M. Cuomo to fundamentally restructure the health care delivery system by reinvesting in the Medicaid program, with the primary goal of reducing avoidable hospital use by 25% over 5 years. The mission of Millennium Collaborative Care is to collaborate and partner with physicians, healthcare providers and community-based organizations on the implementation of 11 DSRIP programs designed by Millennium to positively impact Medicaid patients across Western New York’s eight counties (Allegany, Cattaraugus, Chautauqua, Erie, Genesee, Niagara, Orleans, Wyoming).
