Free Million Hearts® Health Screenings Set for ‘Spirit of WNY Health & Wellness Expo’

1 PM – 4 PM Sat., Oct. 28 at Buffalo Niagara Convention Center

Buffalo, N.Y. – Millennium Collaborative Care, in partnership with Greater Buffalo United Ministries and the University at Buffalo School of Nursing, will host a Million Hearts® health screening event as part of the WUFO/Power 96.5 Health & Wellness Expo set for Sat., Oct. 28, 2017 at the Buffalo Niagara Convention Center. The Expo is free and doors will open at Noon. Adults 18-years of age and older can receive a free Million Hearts® health screening from 1 PM – 4 PM.


Hosted by WUFO and Power 96.5, the “Spirit of New York Health & Wealth Expo” is a multicultural event that will highlight the rich history of WNY and its surrounding areas. The Expo will feature national talk show host Tom Joyner; health and wellness providers serving men, women and children; and will also present a live concert beginning at 6 PM with chart – topping jazz gospel guitarist Tim Bowman, powerhouse contemporary gospel vocal trio Half Mile Home, O’Jays member Eric Nolan and more! In addition to blood pressure screening by UB Nursing Students, Expo attendees will have the opportunity to visit three additional stations including: Personal Health Survey; Height and Weight; along with receiving education and resources for nutrition and exercise.

“Cardiovascular disease which includes heart attacks and strokes is the leading reason for death across our eight Western New York counties,” said Kenneth Lee Gayles. M.D., Cardiologist, Gayles Medical; and Project Champion, Cardiovascular Disease/Million Hearts®.

“The goal of our Million Hearts® events is really to help people understand and control their blood pressure,” added Priti Bangia, Population Health Manager, Millennium Collaborative Care. “This is so important because blood pressure if not treated, can result in people having heart attacks and strokes.”


After completing their testing and screening, a Community Navigator will also assist attendees with checking their eligibility for Medicaid, as well as help them find a primary care provider if they currently do not have one.


Heart disease and stroke are some of the top leading causes of death in the U.S. Statistics show that every 43 seconds someone has a heart attack in the U.S., as well as 1 American dies every 4 minutes from a stroke, according to

Million Hearts® is a national initiative co-led by the Center of Disease Control (CDC) and Center for Medicare and Medicaid Services (CMS), with the goal of reducing the number of heart attacks and strokes by one million.

Millennium Collaborative Care (MCC), is a Performing Provider System (PPS) under New York State’s Delivery System Reform Incentive Payment (DSRIP) Program, a statewide initiative championed by Governor Andrew M. Cuomo to fundamentally restructure the health care delivery system by reinvesting in the Medicaid program, with the primary goal of reducing avoidable hospital use by 25 percent over five years.

The mission of Millennium Collaborative Care is to collaborate and partner with physicians, healthcare providers and community-based organizations to improve the delivery of healthcare to Medicaid clients across Western New York’s eight counties (Allegany, Cattaraugus, Chautauqua, Erie, Genesee, Niagara, Orleans, Wyoming).
