Heading home after a hospital stay can feel like a huge relief. You’re back in familiar surroundings and ready to get back to normal. But recovery doesn’t end when you leave the hospital. Understanding your care plan, managing medications, and following up with your providers can help you recover safely and avoid an unexpected return to the hospital.

“Many hospital readmissions can be prevented when patients have the information, support, and resources they need after discharge,” says Lorna Fitzpatrick, MD, vice president and chief medical officer at Univera Healthcare. “A little preparation before leaving the hospital can make a big difference once you’re home.”

Before You Leave: Get the Details. 

Before heading home, your healthcare team will review your discharge plan. The more familiar you are with it, the more confident you’ll feel managing your care at home. Make sure you understand:

  • How to care for wounds, incisions, or bandages 
  • Any medical equipment you’ll need and how to use it 
  • When follow-up appointments are scheduled 
  • Activities you should avoid or gradually resume 
  • Symptoms that should prompt a call to your doctor 

Don’t Be Afraid to Ask Questions.

“If something isn’t clear, speak up. Understanding your recovery plan is one of the best ways to help prevent complications,” says Fitzpatrick.

Don’t Overlook Your Medications.

 Before you leave, review each medication with your provider and make sure you know:

  • What each medication is for 
  • How much to take and when 
  • What to do if you miss a dose 
  • Possible side effects  

Once you’re home, fill new prescriptions promptly and keep an up-to-date medication list to share with your healthcare providers.

Lean on Your Support System.

 Let a trusted friend or family member know about your recovery plan. They can help with transportation, errands, medication reminders, meal preparation, or regular check-ins. Extra support can provide peace of mind.
 

Keep Your Follow-Up Appointments.

Follow up with your primary care provider and any specialists involved in your care. These visits help ensure your recovery is progressing as expected and allow providers to address concerns early.
