There comes a point for many people when something shifts, and the path forward is no longer clear. It might be a new health diagnosis, a change in mobility, the loss of housing, or the growing realization that someone you love needs more support than you know how to provide. What follows is often uncertainty about where to start or even what options are available. NY Connects exists to meet people in those moments.

Operating across New York State, NY Connects serves as an entry point to long-term services and supports, helping individuals and families understand what resources are available and how to access them. It is designed to reduce confusion in a system that can be difficult to navigate, especially for those encountering such challenges for the first time.

NY Connects supports people of all ages, including older adults, individuals with disabilities, and caregivers. Some reach out with clear needs in mind, such as assistance with home care or transportation. Others are less certain, trying to make sense of a situation that has changed quickly and without much warning. In either of these instances, the role of NY Connects is not to direct, but to provide information and walk people through their options—helping them move forward in a way that reflects their circumstances.

This approach is often described as “No Wrong Door,” meaning that no matter how someone enters the system, they can access support without being redirected or turned away. Whether the first contact is made over the phone, online, or in person through a local partner, the goal remains the same: to take the time to understand what’s needed and connect people with the right resources.

From there, support becomes more specific. NY Connects staff can help individuals understand eligibility for public benefits, including Medicaid and SNAP, and guide them through application processes that can feel overwhelming. They also connect people to community-based services, including housing supports, nutrition programs, caregiver resources, and independent living services that make it possible to remain at home and stay engaged in the community.
