Founded in 1983 as AIDS Community Services, Evergreen Health is a comprehensive healthcare system offering medical, supportive, and behavioral health services. We understand that not everyone has the same access to the basic things they need to be healthy and remain focused on our long-term goal of eradicating health inequities by providing unconditional care.
For more than 40 years, Evergreen has provided support and access to critical services for everyone who walks through our doors, and we will continue to work to overcome obstacles and collaborate with community partners to ensure that everyone in Western New York can lead healthier, happier lives.
Serving over 27,000 patients annually across seven locations, including 17,000 who receive federally qualified health center services, our unique care model treats the whole patient to improve health outcomes for the entire community. We also recognize that a strong culture is the foundation that enables us to continue working toward our long-term goals.
